We understand that arriving at a chiropractic office for the first time can be nerve racking. If you’ve never experienced chiropractic care before, you may not know what to expect when you come to our office for the first time. That’s why our aim is to remove any unnecessary stress or tension and make you feel comfortable during your visit.
Patient Forms

1) Fill out the relevant sections of our Online Intake Form, submit, and they will be downloaded directly to our server!
2) Call 317-848-4408 to schedule, OR pick your appointment time online using our Online Scheduler. You will receive an email confirming your appointment or requesting a change to the next closest 15-minute interval.
Consultation & Examination

Initial visits will begin with a thorough history and detailed examination based on your specific condition. The exam will include neurological, muscle, and orthopedic testing in order to find the root cause of your pain, as well as motion palpation, the gold standard for assessment in functional manual care, to locate mechanical joint dysfunction within the spinal column and extremities.
Report of Findings

Once the information is collected and examinations are performed, Dr. Butwin will give you a detailed report of all findings and answer any questions including:
- How can you help me?
- How often do I need to come in?
- What will my treatment cost?
Treatment

Treatment will normally be provided during your first visit and will be catered to your specific needs and goals. Chiropractic manipulation Active Release Technique, stretching, basic therapeutic exercises, functional rehabilitation, kinesio taping, or various passive modalities may be utilized to first provide you with relief, then fix the cause of your pain, and finally allow you to maintain optimal health and joint function in the long run. After treatment, Dr. Butwin will discuss with you his treatment plan – usually laid out in a two week timeframe. After two weeks, your condition will generally be re-assessed. You will not be forced to pay up front for long-term, multiple-visit contracts. You will not be forced to have X-rays taken unless they are found to be clinically necessary. And finally, you will not be coerced into being treated more than clinically necessary or charged for services that you don’t need.
